Current challenges in laboratory diagnostics of autonomous cortisol secretion in adrenal incidentalomas

چکیده
Adrenal incidentalomas are unexpectedly identified adrenal masses without prior suspicion of the existence disease. The majority benign adenomas (80-90%), and they non-functioning adrenocortical in more than 70% cases. Mild hypercortisolemia is most common finding hormonally active incidentalomas. It defined as autonomous cortisol secretion up to 15% patients. Despite a lot research that anticipated higher cardiometabolic risk patients with secretion, there still no clear consensus on biochemical criteria for an diagnosis This review delineates advantages limitations different laboratory tests recommended

Current Management of Adrenal Incidentalomas
Adrenal “incidentalomas” refer to a group of adrenal masses initially discovered during cross-sectional imaging obtained for other clinical reasons. The majority of incidentalomas are benign nonfunctional adrenal adenomas and can be safely managed expectantly. A subset of adrenal incidentalomas, however, are functional and/or malignant, and these lesions most often require adrenalectomy. The fo...
متن کاملBilateral adrenal incidentalomas differ from unilateral adrenal incidentalomas in subclinical cortisol hypersecretion but not in potential clinical implications.
OBJECTIVE To investigate the possibility of a different prevalence of subclinical Cushing's syndrome (SCS) and potentially related morbidities between patients with unilateral adrenal incidentalomas (UAI) and bilateral adrenal incidentalomas (BAI), as existing data are few and controversial. DESIGN Prospective observational study. METHODS Clinical examination, biochemical tests, and hormona...

As a neurotransmitter or neuromodulator, brain histamine has a variety of physiological roles in brain functions such as hypothalamic- pituitary- adrenal (HPA) activity. Histamine induces the release of ACTH through the activation of hypothalamic neurons containing vasopressin and CRH. Histamine induces the activity of HPA axis directly or indirectly. Endogenous opioids modulate the (HPA) axis ...

متن کاملOvernight dexamethasone suppression of cortisol is associated with radiocholesterol uptake patterns in adrenal incidentalomas.
Low-dose dexamethasone suppression is considered to be the most accurate test for diagnosing subclinical hypercortisolism (1±3). The recommended cut-off point for excluding the presence of Cushing's syndrome is a morning plasma cortisol concentration of less than 50 nmol/l after 1 mg dexamethasone. We read, with interest, the article by Valli et al. (4), who assessed the value of endocrine test...
